Transition Planning
We see residential care, for some service users as being part of a transition to adulthood.
A key term used when developing an individual’s transition from child to adult services is Individualised Transition Plan or ITP.
The plan, including goals and services, is based on the individual needs, preferences, and skills of the person with Autism.
Autism Support and Care transition planning aims to be oriented to life after school, not limited to what has been accomplished before.
The individualised plan includes long range goals and a coordinated set of activities for each.
The transition plan promotes positive movement towards a full and rewarding life.
Many people think of adulthood in terms of getting a job and living in a particular area, but having friends and a sense of belonging in a community are also important. To this end residential care may be appropriate for many people at this time.
